I recommend HostGator because they are easy to work with and the customer service is unbeatable! Then send traffic you receive from your Google ad to the landing page.
After a visitor takes the survey, thank them and send them something free in exchange for their name and email address.
Note it is important that the participants receive something in return for their information. Because in reality nothing is free. This is your way of paying them for taking your survey and providing you with their personal details.
Offer them a free ebook, newsletter, or even a months free membership into one of your programs.
Now here is the beauty of the whole thing. When someone gives you there personal information you get paid. Anywhere from$0.50 to $1.50 per lead. Since you only paid about $0.07 for your Google ad....
